Supporting documents
Upload a document
You must save the form before you can upload documents.
In the cause of death form:
- Go to the Action list (just below the form’s top heading, on the right)
- Select Add document and ‘Go’
- Select the details describing the type of document
- ‘Choose file’ to browse your computer and select the file to upload
- ‘Save’
Print the MCCD or MCCPD
Download, print and sign a copy for the funeral director to allow them to collect the deceased person.
From the Drafts list:
- Tick the box to the left of the person’s name
- Select the Print button (at the bottom of the list)
- Tick the box for the type of document you want to print
- Select ‘Generate document for printing’, which downloads a PDF for printing
