Supporting documents

Upload a document

You must save the form before you can upload documents.

In the cause of death form:

  1. Go to the Action list (just below the form’s top heading, on the right)
  2. Select Add document and ‘Go
  3. Select the details describing the type of document
  4. Choose file’ to browse your computer and select the file to upload
  5. Save

Print the MCCD or MCCPD

Download, print and sign a copy for the funeral director to allow them to collect the deceased person.

From the Drafts list:

  1. Tick the box to the left of the person’s name
  2. Select the Print button (at the bottom of the list)
  3. Tick the box for the type of document you want to print
  4. Select ‘Generate document for printing’, which downloads a PDF for printing
