Domestic relationship (defacto): put ‘Neither’

Children of this relationship

Include any deceased children

Previous relationships

Don’t include previous:

• registered relationships
• civil unions

Upload the court order

You must save the form before you can upload documents.

In the adoption notification form:

1. Save the form
2. Go to the Action list (just below the form’s top heading, on the right), select Add document and ‘Go’
3. Select the details describing the type of document
4. ‘Choose file’ to browse your computer and select the file to upload
5. ‘Save’
